Registration will be held on Saturday, February 27th at Indianola
Preschool. Registrations will not be accepted prior to February 27th.

After printing the form above, fill it out and
bring it with you to registration. Forms will also be available on registration
day at IPS. If you have a child you would like to enroll please come to
Indianola Preschool at the appropriate time listed below: |

| Important Registration Details: |

A
completed Registration Form and FULL payment of registration fees (and any
outstanding balances due) must be received at the time of registration in order
to reserve your child's spot. Please come prepared with a check or
cash! |

Registrations will be accepted on a first come first serve basis
according to the times listed above. |

A parent
or family member must register their child at the preschool. Registrations will
not be accepted by friends. |

Registrations will continue to be accepted after February 27th, to
fill any remaining openings. |

Please
enter through the classroom door on the east side of the building. |

Registration Fee plus |
| 2. One
Month's Tuition (applied to last month's tuition) |
*Tuition scholarships are available through Warren County Empowerment
Board.
*If a class does not have sufficient registration, the class will
be terminated

child will be attending IPS for the 2010-2011 school year, please print the
necessary paperwork to complete your child's preschool file.

*We also need to have a valid Iowa Certificate of
Immunization form. This can be obtained by contacting your doctor's office.
All forms are to be completed by July 1, 2010.
